401(k)

Provided by Lincoln Financial – 47 funds to choose from. Three plans to choose from – Two are Safe Harbor Plans. Gwen O’Connell Pension Consulting, Inc. of Eugene is the Third party administrator.

125/129 Cafeteria Plan

Total Administrative Services Corporation is the administrator, allows pretax withholdings for dependent care. medical, dental, and vision expenses not covered by insurance. Group health insurance premiums.

Group Health Insurance

Cardinal will work with client’s licensed insurance agent or assist client in finding an agent to obtain a group plan for the client. Cardinal can be billed directly for the plan and will take care of the employee withholdings and plan administration.

Group Life Insurance

Transamerica Insurance – Employees can obtain up to $100,000 of insurance with no question or medical exam; up to $300,000 with minimal questions and no exam.

Dental Insurance

Cardinal Plan, administered by ARM: 100% Preventive Services, 80% Basic Care, 50% Major Services, $500 Orthodontic; $1000 Plan Limit per Person each Calendar Year with $50 annual deductible/person.

Vision Insurance

Cardinal Plan, administered by ARM: $15 co-pay for exam once a year, $15 co-pay for lenses and contacts every 2 years.
